Athletic Forms

To complete the Athletic Clearance Process, all original forms must be dropped off at the DLA Finance Window.

Original forms include:

- 21-22 Athletic Clearance Packet
- Sports Physical Evaluation Form (Included in Packet. Doctor may also use their own clearance form.)

Covid-19 Waivers (can be completed digitally and e-mailed to Ms. Mendoza)

- Covid Risk Waiver
  - Asunción de Exención de Riesgo por Parte del Deportista
- Student Athlete Covid Guidelines
  - Pautas de Covid para Estudiantes Deportistas

Please note: An online account must be created by the athlete on https://www.athleticclearance.com/. If this account is not created and completed at the time of form drop off, the scholar will not be cleared.

To drop off forms, please make appointment before visiting the school by calling (760) 291-2515.

Finance Window hours are as follows:

Monday: 8:00am – 4:15pm
Tuesday: 8:00am – 4:15pm
Wednesday: Closed
Thursday: 8:00am – 4:15pm
Friday: Closed
